Annex 1 - Mandatory Conditions
1. No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
2. Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3.-(1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule

Annex 3 - Conditions attached after a hearing by the licensing authority
1. The Licence Holder shall have installed and operating and maintain CCTV equipment on the premises. All CCTV tapes shall be retained and not re-recorded for a period of 31 days.
2. Notice of CCTV operating at the premises shall be displayed on the premises.
3. Age-limit signs shall be displayed within the premises.
4. The Licence Holder shall provide closed waste bins outside the premises and shall at the close of business ensure the forecourt is clear of all litter.
5. Regular staff training and retraining shall be undertaken and recorded and staff training records shall be kept on the premises and available for inspection.
6. Challenge 21 Scheme shall be in force on the premises.
7. Refusal Register to be maintained on the premises and available for inspection.
8. During all authorised sales of alcohol, the Licence Holder shall be present on the premises.
