Annex 1 - Mandatory Conditions
1. No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
2. Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3.-(1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule
1. The ordering by sale/supply of any alcohol product to anybody under the age of 18 is strictly forbidden.
2. Any person placing an order by telephone or having alcohol delivered to them, will always have to provide some form of acceptable and recognised means of proving their age: REFUSAL TO PROVIDE PROOF OF AGE DETAILS WILL RESULT IN AN ORDER, SALE OR DELIVERY BEING REFUSED.
Orders for sale/supply of alcoholic products .
3. No person under the age of 18 will be employed as a sales assistant who is involved in the operation of this business and has a responsibility for the ordering, sale/supply of alcohol at the licensed premises.
4. Sales assistants taking orders for alcoholic products over the telephone will always ask for the persons date of birth who is placing the order,
if a date of birth cannot be established with the telephone order, then the sales assistant will REFUSE the order outright.
5. All sales assistants who have responsibility for taking orders for the sale/supply of alcoholic products will have regard to the Challenge 25 scheme when invoicing the order for delivery.
6. Sales assistants taking the orders for alcoholic products over the telephone will also REFUSE the order outright to someone who in their opinion is already drunk on the telephone.
7. If a sales assistant refuses any order for EITHER age reasons or drunkenness, the details of the person (where known), their address (where known) and the date and time of the order will be entered in a separate refusal book, to be retained at the licensed premises for inspection at any time by a Police Constable or an authorised officer of the Licensing Authority.
8. Every time an order for any alcoholic product is processed, then an invoice will be produced by sales assistants. Every invoice will contain the following ;
Personal details of the person placing the order which the proprietor thinks appropriate, but which must include the name, full postal address telephone number and the date of birth of that person.
The quantity, description, and price of the alcoholic products ordered.
The date and time that order took place.
The business telephone number used to place the order and where the order was accepted.
How the invoice total was, or will be paid for, by credit/debit card with the order or cash on delivery.
Dispatch / delivery of alcoholic products previously ordered .
9. Delivery of ANY alcoholic products shall ONLY be dispatched from this address (the licensed premises, NO OTHER).
10. All alcoholic products will be sealed vessels, not opened in any way.
11. Delivery of ANY alcoholic products shall ONLY take place when an order for those products has previously taken place in a manner described before.
12. No person under the age of 18 will be employed as a driver of a vehicle who is involved in the operation of this business and has responsibility for the delivery of alcoholic products from these licensed premises.
13. Aside from the ordering, if the driver / person delivering is in any DOUBT as to the age of that person, then he will ask for proof of age accepting only valid and accredited cards/documents recognised by West Yorkshire Police and West Yorkshire Trading Standards to avoid any sales/supplies to minors.
14. Aside from the ordering, if there is still doubt as to the age of the that person, or if proof of age cannot be produced when asked for, then the delivery WILL BE REFUSED and the complete alcohol order will be RETURNED to the licensed premises from where it was dispatched.
15. All persons involved in the supply and sale of alcohol under the provisions of this licence will ALWAYS familiarise themselves and adopt the Challenge 25 Scheme.
16. If a delivery is refused under the provisions of this licence, for any reason , full details will be recorded in a refusals book which will be kept at the licensed premises and will be available at any time for inspection by West Yorkshire Police or any other appropriate licensing authority.
17. Any driver/person delivering will also REFUSE to deliver to anybody who in their opinion appears to be DRUNK at the time of delivery. Such details will also be recorded in the refusals book, and the order WILL be returned to the licensed premise from where it originated.
18. Sales will only be made to organisations registered with the company and not to individual private residential dwellings.
19. CCTV system to a specification required by West Yorkshire Police as per Home Office guidelines will be installed and utilised at the premises with sufficient storage capacity and requirement to store the previous 31 days footage. Staff will be trained in the use of the system to ensure rapid data retrieval and down load when required by Police or Local Authority Officers. CCTV to be on a 24 hour basis and at least one member of staff to who is trained in downloading from the system is on the premises when it is open.
20. No members of the public will be allowed on the Premises
21. The Fire Alarm and Emergency Lighting Systems are to be tested weekly and a written record of these tests is to be kept. Any faults identified and the relevant actions taken to rectify them are to be noted in the record.
22. An appropriate first aid equipment box shall be provided at all times.
23. All exit routes must be kept clear and remain free from obstruction at all times.
24. At the end of business every day/night, the pavement to the frontage (and sides) of the premises shall be cleared of litter and waste.

Plans have not been re-issued with this licence/certificate as they remain unchanged from the previous application. You should now attach those plans to this licence/certificate as you can be required to produce them as part of the licence/certificate by the police or other authorised person.
