Annex 2 - Conditions consistent with the Operating Schedule
1. The Premises Licence holder will ensure that an Incident Report Register is maintained on the premises to record incidents such as anti social behaviour and ejections from the premises.
The register will contain consecutively numbered pages in a bound format.
The details of the incident must include the date, time and location of the incident, details of the nature of the incident, the names to whom the incident was reported, the names of any other staff involved, the names and numbers of any Police officers attending, names and addresses of any witnesses and confirmation of whether there is CCTV footage of the incident.
The incident report register shall be maintained and available for inspection by West Yorkshire Police and Local Authority Enforcement Officers
2. CCTV
2.1 A CCTV system to a specification required by West Yorkshire Police and in accordance with current Home Office Guidelines be installed and utilized at the premises with a sufficient storage capacity to store a minimum of 31 days footage;
2.2 Equipment shall be operated and maintained in good and clear working order and recordings shall be kept in date order, numbered and retained for a period of at least 31 days, or as otherwise recommended by the Police;
2.3 Staff shall be trained in the use of the system to ensure rapid data retrieval and download when required by Police or Local Authority Enforcement Officers;
2.4 CCTV shall be active on a 24 hour basis and at least one member of staff who is trained in downloading from the system shall be on the premises during trading hours.
3. Training
3.1 New staff shall receive induction training at the commencement of their employment at the premises, including how to deal with anti-social behavior and refusing to serve drunken customers; Existing staff shall also be subjected to the same level of training;
3.2 Staff refresher training shall take place on a quarterly basis;
3.3 All staff training shall be recorded and training records maintained and available immediately for inspection by West Yorkshire Police and Local Authority Enforcement Officers upon request .
4. There shall be no alcohol permitted on the premises at any time.
5. At the end of business every day/night the pavement to the frontage of the premises shall be cleared of litter and waste and cleansed of any urine, vomit or similar foulings.
6. Facilities for depositing rubbish outside the premises must be made available.
7. An appropriate first aid equipment box shall be provided at all times.
