Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 - Conditions consistent with the Operating Schedule

Conditions Endorsed on Justices' On-Licence
1. Intoxicating liquor may only be sold or supplied to the following persons:
a) Persons who are members of the said club being persons duly admitted to the said Club or have been duly admitted to membership in accordance with the rules of the Club and who have been members for at least 28 hours or whose nomination or application for membership was made at least 28 hours prior to admission;
b) A guest of such a member, introduced by him in accordance with the Rules of the Club;
c) Persons employed at the said premises.
2. No persons under the age of 18 years shall be eligible for membership of the said Club.
3. Subject to rule 4 below, no persons under 18 years of age shall be admitted to the Club.
5. Subject to rule 6 below, guests of members (including Juveniles) shall 'sign in' their visit in a book kept for that purpose and be available for inspection by a police constable in uniform on demand.
6. On the actual day or days on which any match shall be played at the Club premises all the visiting members of any opposing club or team and the referee or referees of any such matches shall be regarded as visitors of the club including the right to be supplied with or sold intoxicating liquor for consumption on the premises, but all visitors (including juveniles) shall 'sign in' their visit in a book kept for the purpose and be available for inspection by a police constable on demand.
7. One member may only introduce 3 guests at any one time or day.
11. A complete list of members of the Club shall be maintained and kept available on the premises to be inspected by a police constable in uniform on demand if required.
13. To comply with the recommendations of the Fire Officer.
Operating Schedule Conditions
Noise from amplified and non-amplified music, singing and speech arising from regulated entertainment at the premises [between the hours of 23:00 and 07:00] shall not be audible inside habitable rooms of noise sensitive properties in the vicinity.
Informative Note:
Licensees are advised to carry out a simple “sound check” outside the nearest noise sensitive property by listening to the music etc. coming from regulated entertainment. If the music etc. is clearly audible then it is likely that this condition is being breached. Steps should then be taken to reduce the volume of the noise.
For the purpose of providing an objective standard to assess whether this condition is being complied with or not, noise shall be considered not to be audible if:
) the measurement of sound when music etc is on (expressed as LAeq, 1min) does not exceed the measurement of sound (expressed as LA90, 5min) when the music is off, and
) the measurement of sound in each 1/3rd octave band between 40Hz and 160 Hz when music is on (expressed as L10, 1min) does not exceed the measurement of sound in the same 1/3rd octave bands between 40Hz and 160Hz when the music is off (expressed as L90, 5min)
All measurements to be taken within the habitable room normally occupied at the time in question, using a Type 1 integrating-averaging sound level meter compliant with BS EN 60804, with either windows open for normal ventilation or closed, whichever situation causes greater noise disturbance.
All Live Entertainments and any DJ with Discos shall only operate within the rear Function Room.
Speakers used to relay amplified music, singing and speech provided as part of the regulated entertainment shall be played through a sound amplification system incorporating a sound limiting device that has been installed and ser to the express satisfaction of the Responsible Authority for Public Nuisance (Wakefield MDC's Environmental Protection Team). The sound-limiting device shall be retained and maintained throughout the life of the licence and no alteration to the approved setting shall be made without the express approval of the Responsible Authority.
The rear fire-exit doors to the Function Room shall be kept closed during the course of any regulated entertainment.
The Fire Alarm and Emergency Lighting Systems are to be tested weekly and a written record of these tests is to be kept. Any faults identified and the relevant actions taken to rectify them are to be noted in the record.
All exit routes must be kept clear and remain free from obstruction at all times.
Building Control recommend that the following occupancy levels are not exceeded:
Bar 100 persons
Function Room 150 persons
American Pool Room 20 persons
Snooker Room 60 persons
The Club shall continue to have controlled access at the entrance to ensure that only members and guests are admitted.
Any person on the premises who appears to be under 18 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
The Club shall continue to issue the different colour membership cards for 'junior members', i.e. those under 18 years of age. They shall be restricted to the snooker and pool areas and not allowed to the part of the bar that serves alcohol.
CCTV shall continue to be used on the premises during the hours of trading. Cameras should be kept in full working order and the recordings shall be retained for a length of time in accordance with local police advice.
