Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Each individual carrying out a security activity must be licensed by the Security Industry Authority.
A record shall be kept of the names and registration number of such persons and this record shall include the circumstances including full details of all events that take place during their period of duty.
Annex 2 - Conditions consistent with the Operating Schedule

Alcohol shall not be sold or supplied except during permitted hours.
The above restrictions do not prohibit:
a) During the first twenty minutes after the above hours the consumption of the alcohol on the premises;
b) During the first twenty minutes after the above hours, the taking of the alcohol from the premises unless the alcohol is supplied or taken in an open vessel;
c) During the first thirty minutes after the above hours the consumption of the alcohol on the premises by persons taking meals there if the alcohol was supplied for consumption as ancillary to the meals;
d) Consumption of the alcohol on the premises or the taking of sale or supply of alcohol to any person residing in the licensed premises;
e) The ordering of alcohol to be consumed off the premises, or the despatch by the vendor of the alcohol so ordered;
f) The sale of alcohol to a trader or club for the purposes of the trade or club;
g) The sale or supply of alcohol to any canteen or mess, being a canteen in which the sale or supply of alcohol is carried out under the authority of the Secretary of State or an authorised mess of members of Her Majesty’s naval, military or air forces;
h) The taking of alcohol from the premises by a person residing there; or
i) The supply of alcohol for consumption on the premises to any private friends of a person residing there who are bona fide entertained by him at his own expense, or the consumption of alcohol by persons so supplied; or
j) The supply of alcohol for consumption on the premises to persons employed there for the purposes of the business carried on by the holder of the licence, or the consumption of liquor so supplied, if the liquor is supplied at the expense of their employer or of the person carrying on or in charge of the business on the premises.
Operating Schedule Conditions
1. Promotional leaflets (flyers) shall not be distributed to the public outside the premises (whether immediately outside or some distance away) except with the prior written approval of the Licensing Authority, and under the terms of a litter control plan approved by the Responsible Authority for Public Nuisance (Wakefield MDC’s Environmental Protection Team).
2. CCTV shall continue to be used on the premises. Cameras should be kept in full working order and the recordings shall be retained for a length of time in accordance with local police advice.
3. Children (as defined in the Licensing Act 2003) shall not be allowed on the premises under any circumstances.
Special Conditions
Striptease, Lap Dancing, Table Dancing and Similar Entertainment
4. The authorised entertainment must only take place in designated areas approved by the Council;
5. Customers and dancers must be properly supervised at all times;
6. An appropriate number of stewards agreed in writing with the Council, must be provided to properly supervise the premises and maintain order;
7. House rules should be developed and actively promoted to control the behaviour of the dancers and customers;
8. Performers shall only perform in the area(s) of the premises which are licensed for public entertainment;
9. Patrons must not be permitted to participate in the entertainment;
10. No persons other than the performers, should be in the public area of the premises in a state of undress.
11. No performer should perform any sexually explicit or lewd act;
12. No performance shall involve the provocative use of live animals or other props;
13. No performance shall involve any physical contact between each performer;
14. Suitable changing rooms with restricted access should be provided for performers, separate from customers and other staff, where they may prepare for their performance;
15. Safe and controlled access to the dressing room for performers must be maintained at all times when the performance is taking place and immediately afterwards;
16. No person under the age of 18 years shall be permitted to view the entertainment;
17. No performers and other employees on the premises during the authorised entertainment shall be under 18 years of age;
18. A notice shall be displayed at each entrance to the premises in a prominent position so that it can be easily read by persons entering the premises in the following terms:
'NO PERSONS UNDER 18 WILL BE ADMITTED'
19. A notice should also be displayed at the main entrance informing customers of the house rules that affect them and a warning that entertainment involving unclothed male and/or female performers is provided within the premises;
20. No advertising should be outside the premises, of striptease, lap dancing, table-dancing or similar entertainment, which may give rise to offence.
21. No part of the performance shall be visible from outside the premises.
Annex 3 - Conditions attached after a hearing by the licensing authority
1. Two SIA registered door staff to be on the premises at all times of trading between 20.00hrs and close of business.
2. A radio link will remain in place between Heaven and CCTV control room and Pontefract Police Station.
3. CCTV recordings will be on hard drive and kept for a minimum of 30 days.
4. Challenge 21 policy will be adopted and appropriate signs will be displayed on the premises.
