Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Each individual carrying out a security activity must be licensed by the Security Industry Authority.
A record shall be kept of the names and registration number of such persons and this record shall include the circumstances including full details of all events that take place during their period of duty.
Annex 2 - Conditions consistent with the Operating Schedule
1. At the end of business every day/night the pavement to the frontage of the premises shall be cleared of litter and waste and cleansed of any urine, vomit or similar fouling.
2. The licensed premises will store and dispose of business waste correctly and legally. The Designated Premises Supervisor must ensure that the waste is prevented from seeping or spilling from where it is stored.
3. A suitable CCTV system will be maintained and be operational on the premises at all times when licensable activities are being carried out.
4. Any person on the premises who appears to be under 21 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
5. All emergency lighting and/or fire alarm system to be checked weekly and a record kept. Any faults identified and the relevant action taken to be noted.
6. Adequate external lighting shall be provided at all times. Exit sign boxes must be lit by both primary and emergency lighting.
7. All exit routes must be kept clear and remain free from obstruction at all times.
8. Suitably trained First Aid staff shall be available at all times that the premises are open to the public.
9. The premises shall operate a suitable ventilation and extraction system. The system will be cleaned and maintained to the manufacture's instructions to prevent unwated odours occurring.
10. Prominent, clear notices shall be displayed at all exits requesting staff to respect the needs of local residents and leave the premises and area quietly.
11. A Register shall be maintained at the licensed premises, showing the names, addresses and up-to-date contact details for the Designated Premises Supervisor and all personal licence holders.
12. Any customers who commit acts of anti-social behaviour shall be removed from the premises. Such customers shall be excluded from the premises if further incidents occur.
13. No adult entertainment shall take place on the premises.
14. Electrical installations shall be inspected on a periodic basis (atleast every 5 years) by a suitably qualified and competent person. Inspection records/ certificates shall be retained and will be made available on request to Authorised Officers.
15. Adequate and appropriate First Aid equipment and materials shall be available on the premises.
16. No nuisance shall be caused by noise or vibration emanating from the premises. Licensable Activities shall be conducted and the facilities for Licensable Activities shall be designed and operated so as to prevent the transmission of audible noise or perceptible vibration through the fabric of the building or structure to adjoining properties.
