Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 - Conditions consistent with the Operating Schedule
1. CCTV shall be used on the premises. Cameras must be kept in full working order and the recordings shall be retained for a length of time in accordance with local police advice.
2. Any person on the premises who appears to be under 18 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
3. No adult entertainment shall take place on the premises.
4. Litter bins shall be available for customer use outside the premises at all times the premises are trading.
5. Litter bins so provided at 4 above shall be emptied at regular intervals and at the end of trading every day.
6. At the end of business every day/night the pavement to the frontage (and sides) of the premises must be cleared of any litter and waste and cleansed of any urine, vomit or similar fouling.
Annex 3 - Conditions attached after a hearing by the licensing authority
1. All glasses to be of a plastic or polycarbonate manufacture.
2. One door supervisor to be on duty at all times that the premises are trading and a second door supervisor to start at 01.00 hours.
3. The premises to adopt the Challenge 21 standard.
4. All bar staff (other than Personal Licence Holders) to be put through the BII's Responsible Retailing of Alcohol Course prior to them starting work at the premises.
5. The hours to be revised to 04.00 for Licensable Activities Mondays to Saturdays and 04.30 close and 03.00 for Licensable Activities and 03.00 close on Sundays.
6. The premises to comply with the "Secure by Design" standard set by West Yorkshire Police within 4 weeks of the hearing date.
7. The CCTV at the premises to be installed and maintained to teh satisfaction of West Yorkshire Police (in accordance with Secure by Design).
8. The premises to take part in the local radio link scheme.
9. The maximum capacity for the premises to be limited to 70 persons or such numbers agreed with the Fire Officer, such number to exclude any staff.
10. The Premises Licence Holder will maintain a refusal/incident book.
11. Staff training records shall be kept and made available for inspection.
