Annex 1 - Mandatory Conditions
1. No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
2. Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3.-(1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule
1. The Fire Alarm and Emergency Lighting Systems are to be tested weekly and a written record of these tests is to be kept. Any faults identified and the relevant action taken to rectify them are to be noted in the record.
2. All exit routes must be kept clear and remain free from obstruction at all times.
3. A CCTV monitoring system shall be installed and maintained. Images shall be recorded and retained for at least 28 days and shall be made available to the police and authorised officers on request.
4. Notices shall be displayed instore that it is an offence for people under 18 to purchase alcohol.
5. No deliveries shall take place between the hours of 2300 and 0600 the following day.
6. Any person on the premises who appears to be under 21 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
7. The store shall continue to maintain a refusals log book and keep the log fully up to date with reference to recording those suspected of being under the age of 18.
8. The store management shall regularly inspect the refusals log to ensure that the log is being properly maintained.
