Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule
1. A minimum of three members of management shall be assigned to the store, all of whom shall hold a personal licence.
2. A minimum of of two managers shall be on site at all times the premises are open to the public.
3. The premises shall have internal and external CCTV cameras installed. The CCTV system shall be registered in accordance with the data protection act. The system shall be capable of continuously recording and copies of such recordings shall be kept for a period of not less than 31 days and handed to a police constable or authorised person on request. The system shall be maintained in working order & recordings shall be made for each training period conducted at the premises.
4. Any person on the premises who appears to be under 21 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age. The proof of age shall be assertained by the inspection of a recognised form of photographic identification such as a passport, photocard driving licence, proof of age card or any other form of identification agreed with the police.
5. Notices shall be prominently displayed advising customers of the challenge 21 (or similar scheme) policy.
