Annex 1 - Mandatory Conditions
1. No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
2. Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3.-(1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule

Operating Schedule Conditions:
1. CCTV shall be used on the premises. Cameras must be kept in full working order and the recordings shall be retained for a length of time in accordance with local police advice.
2. The Fire Alarm and Emergency Lighting Systems are to be tested weekly and a written record of these tests is to be kept. Any faults identified and the relevant actions taken to rectify them are to be noted in the record.
3. All exit routes must be kept clear and remain free from obstruction at all times.
4. Adequate external lighting shall be provided at all times.
Annex 3 - Conditions attached after a hearing by the licensing authority
1. There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 18 years to produce one of the following forms of identification
(a) a recognized proof of age card accredited under the British Retail Consortiums Proof of Age Standards Scheme (PASS).
(b) photo driving licence
(c) passport
(d) any other means approved and accredited by West Yorkshire Police and/or West Yorkshire Trading Standards Service
2. All point of sales staff shall undergo annual training in the above policy with a record kept of the date of training signed by the member of staff and the trainer. Each staff training entry shall be retained for a period of 24 months from the date of completion. The record of training shall be made available for inspection by Police Officers, Licensing Officers, Officers of West Yorkshire Trading Standards Service or other parties acting on their behalf.
3. A notice or notices shall be displayed in and at the entrance to the premises where they can be clearly seen and read and shall indicate
(a) that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age
(b) that proof of age may be requested at the premises and listing the means for proof of age
4. A refusal register is to be maintained on the premises to be kept fully up to date with such information which may be requested by West Yorkshire Police and shall be available for inspection at all times. The register shall be in a bound pages format with each page consecutively numbered and each entry is to be dated and signed by the staff member making the refusal and also by the Designated Premises Supervisor. The entry and the register are to be retained for a period of 24 months from the date of completion. The register is to be produced on request to a Police Officer, Licensing Officer, or Officer at West Yorkshire Trading Standards Service.
5. Any CCTV evidence of a refusal shall be cross referenced to the specific refusal.
