Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
The admission of children to an exhibition of a film where the film classification body or this Council has determined the classification must be restricted in accordance with that classification. The film classification body is the British Board of Film Classification. In this condition children means persons under 18 years of age.
Films must be classified in the following way:
U - Universal. Suitable for audiences aged four years and over.
PG - Parental Guidance. Some scenes may be unsuitable for young children.
12A - Passed only for viewing by persons aged 12 years or older or persons younger than 12 when accompanied by an adult.
15 - Passed only for viewing by persons aged 15 years and over.
18 - Passed only for viewing by persons aged 18 years and over.
Immediately before each exhibition at the premises of a film passed by the British Board of Film Classification there shall be exhibited on screen for at least five seconds in such a manner as to be easily read by all persons in the auditorium a reproduction of the certificate of the Board or, as regards a trailer advertising a film, of the statement approved by the Board indicating the classification of the film.
Where a programme includes a film recommended by the licensing authority as falling into the 12A, 15 or 18 category no person appearing to be under the age of 12 and unaccompanied, or under 15 or 18 as appropriate, shall be admitted to any part of the programme; and the licence holder shall display in a conspicuous position a notice in the following terms:
PERSONS UNDER THE AGE OF [INSERT APPROPRIATE AGE] CANNOT BE ADMITTED TO ANY PART OF THE PROGRAMME.
Where films of different categories form part of the same programme, the notice shall refer to the oldest age restriction.
This condition does not apply to members of staff under the relevant age while on-duty provided that the prior written consent of the person's parent or legal guardian has first been obtained.
Each individual carrying out a security activity must be licensed by the Security Industry Authority.
A record shall be kept of the names and registration number of such persons and this record shall include the circumstances including full details of all events that take place during their period of duty.
Annex 2 - Conditions consistent with the Operating Schedule

Alcohol shall not be sold or supplied except during permitted hours.
In this condition, permitted hours means:
On New Year’s Eve from the end of permitted hours on New Year’s Eve to the start of permitted hours on the following day (or, if there are no permitted hours on the following day, midnight on 31 December).
Special Hours Certificate
In relation to the morning on which summer time begins, the substitution of references to three o’clock in the morning for references to two o’clock in the morning.
Operating Schedule Conditions
All external windows and the entrance inner lobby to the room/s where regulated entertainment is being provided, shall be closed at 23.00 hours and shall remain closed during the course of the entertainment, other than for normal access and egress through lobby doors.
Speakers used to relay amplified music, singing and speech provided as part of the regulated entertainment shall not be positioned outside the premises, nor internally, but directing out through external doors, windows and similar openings to the premises.
Promotional leaflets (flyers) shall not be distributed to the public outside the premises (whether immediately outside or some distance away) except with the prior written approval of the Licensing Authority, and under the terms of a litter control plan approved by the Responsible Authority for Public Nuisance (Wakefield MDC’s Environmental Protection Team).
At the end of business every day/night, the pavement to the frontage (and sides) of the premises shall be cleared of litter and waste and cleansed of any urine, vomit or similar foulings.
The first floor fire escape door and external, metal staircase must not be allowed to be used by customers except in an emergency. This is to prevent customers falling on the stairs and to prevent customers dropping bottles or glasses from the fire escape into the White Horse Yard.
Prominent, clear notices shall be displayed at all exits requesting customers and staff to respect the needs of local residents and leave the premises and area quietly.
The Designated Premises Supervisor shall continue to remain a member of the Local Pubwatch scheme for as long as such scheme remains in existence.
Any person on the premises who appears to be under 21 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
CCTV shall continue to be used on the premises. Cameras should be kept in full working order and the recordings shall be retained for a length of time in accordance with local police advice.
Text and/ or radio pagers, where already used shall continue to be used for any additional hours the premises remains open to the public. The text/ radio pagers shall be maintained in working order at all times.
The pager link shall be activated, made available to and monitored by the designated premises supervisor or a responsible member of staff at all times that the premises are open to the public.
Any police instructions/ directions shall be complied with whenever given.
All instances of crime or disorder shall be reported via the text/ radio pager link by the designated premises supervisor or a responsible member of staff to an agreed police contact point.
All glasses that are used on the premises shall continue to be either made from toughened glass or plastic
All glasses that are used on the premises, when considered necessary during particular events (e.g. during sporting events etc), shall be made of either toughened glass or plastic.
