Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Each individual carrying out a security activity must be licensed by the Security Industry Authority.
A record shall be kept of the names and registration number of such persons and this record shall include the circumstances including full details of all events that take place during their period of duty.
Annex 2 - Conditions consistent with the Operating Schedule
1. A tamper resistant CCTV system shall be installed, maintained in working order and operated at the premises and must be fit for purpose by complying with the following conditions:
(a) The system shall be used to record during operational hours.
(b) A secure locked area will be provided for the storage of CCTV recordings for upto 28 days.
(c) The system shall be capable of monitoring and recording satisfactory colour images from each camera.
(d) The system will be capable of monitoring and recording entrances and exits, and areas not easliy observed by staff.
(e) Recordings will be made available at all times to the Licensing Authority and all Responsible Authorities.
(f) Signage advising of CCTV being in operation at the premises will be clearly displayed throughout.
2. The Designated Premises Supervisor shall continue to remain a member of the Local Pubwatch scheme for as long as such scheme remains in existence.
3. Any person on the premises who appears to be under 21 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age.
4. Prominent, clear notices shall be displayed at all exits requesting customers and staff to respect the needs of local residents and leave the premises and area quickly and quietly.
5. No persons under 18 years of age will be permitted entry to the premises.
6. No adult entertainment shall take place on the premises.
