Annex 1 - Mandatory Conditions
No supply of alcohol may be made under this Premises Licence:
a) at a time there is no designated premises supervisor in respect of the premises licence and,
b) at a time when the designated premises supervisor does not hold a personal licence or the personal licence is suspended and,
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Each individual carrying out a security activity must be licensed by the Security Industry Authority.
A record shall be kept of the names and registration number of such persons and this record shall include the circumstances including full details of all events that take place during their period of duty.
Annex 2 - Conditions consistent with the Operating Schedule
1. The Fire Alarm and Emergency Lighting Systems are to be tested weekly and a written record of these tests is to be kept. Any faults identified and the relevant actions taken to rectify them are to be noted in the record.
2. All exit routes must be kept clear and remain free from obstruction at all times.
3. Any person on the premises who appears to be under 18 years of age attempting to purchase alcohol from the establishment shall be challenged to produce photographic evidence to ascertain their proof of age. This either be a passport, a photographic drivers licence, or a proof of age card carrying a 'PASS' logo.
4. A CCTV system shall be maintained in proper working order, such a system shall ensure coverage of all entracnes and exits to the licensed premises internally and externally. The CCTV system shall continuously provide recording facilities for each camera, such recording shall be retained for a period of two months and shall be supplied to the licensing authority or the police officer on request. The CCTV system shall be in operation at all times the premises are in use.
